2. Please complete all of these online forms and send them to us by clicking the SUBMIT button. Do not complete the Fertility Questionnaire unless it applies to your question.
3. Once your consult is purchased, we will email you to arrange a time to speak with you.

15 Minute Consultation:

• For specific questions about an herb or supplement,

• Or any Chinese medical advice pertaining to a recent, acute issue.
